After asking John KC5LK who is the WAZ award manager I received the following description how to apply 5BWAZ via eQSL. With his permission I quote here his reply:

"Yes, you can now apply for the 5 Band WAZ.

Your WAZ was all eQSL and you can use all eQSL or a mixture of Paper and eQSL for this award as well. If you want to use eQSL I ask that you make the following changes to the QSL selection process. If you remember there is are two "Filter By" options when you select the eQSLs that you submit. Select in any order 10M, 15M, 20M, 40M, and 80M and then go to the bottom of the page and next to the "Send to CQ" link there is a drop box select 5 Band or 5B and then click "Send to CQ". Return to the to of the page and select another band. Repeat until all five bands are selected. Do not waste your time clicking the "Print the Application" as it will not bear any relationship to what you send me. I usually check eQSL once a week to check for new applications, send me an email telling me that you sent me another application and all the particulars of the application. After I review the application, I will send you a rely that will list the number of zones and fees for the application and any other information that you may need."

I did everything as described and it worked without any problems.

Perhaps tihis is useful for others interested in 5BWAZ application via eQSL.

WAZ base application was no problem with eQSL. Now I would try the application for 5 Band WAZ. But unfortunately I can only select 5 QSLs per Zone and these are often on WARC bands or on the same bands. So in most cases I can apply only for 2 QSLs per Zone. There is a "more..." line on most of my zones but I cannot open or select more QSLs.
What can I do?
