I'm not sure the proper way to handle callsign mismatches when the modifier is the problem. I'll have a QSO with W1AW (for example) and receive an eqsl from W1AW/QRP. Similarly, I'll have another QSO say, W1AW, but receive an eqsl from W7/W1AW.
In both cases there is no matching call in my log. Is it appropriate to update the log or reject the QSL?

You should reject the QSL with an explanation of why.
If W1AW is operating as W7/W1AW, he should be signing his QSOs his QSOs with that callsign so that you will log it correctly. Or, maybe he was operating as W1AW but uploaded his log into his W7/W1AW by mistake.
Either way it needs to be fixed at his end.
